Remove Battery
Hold down the Home and Back arrow keys (same time)
Insert battery
Tap the end call key (red phone key)
White screen appears and asks "Erase User Data?"
Tap the Action button then Menu
"Hands Free Activation" will begin
Provisions by itself

** Remind Customer beforehand they will lose everything
except what's on the SD card (contacts, apps, settings, etc.)**
